DATA PROTECTION AND PRIVACY POLICY

PILATES

A paper or digital health form is to be completed by all one to one or class participants on joining a class or session and up-dated every year. 

 

Your contact details are collected so that Balanced Massage can contact you to inform you of any last minute cancellations or re-scheduling of classes due to illness or bad weather, of future course dates or new classes we are holding.

 

Your medical information is collected only so that the class instructor is aware of any medical conditions that you have that may impact on your safe attendance at classes and allow the instructor to modify exercises on an individual basis. We are required by law to keep these records for a minimum of seven years after your last attendance at a class.

 

Health forms are kept by your Instructor in a folder that may be taken to each class and in between classes is stored in a locked case or cabinet. In transit these forms are kept with the Instructor and not left unattended. In most circumstances a digital replica of your form will be made and the original version shredded. The digital document will be stored on a password protected laptop on iCloud. If you stop attending classes your health form will be deleted after seven years. Telephone numbers are kept on a password protected laptop and phone.

Your medical data is only seen by your individual instructor, except where a colleague is covering a class in the event of sickness or holiday. In this case the instructor covering the class will have access to your health form so that the class can be conducted safely and so that they have your contact details in case of last minute cancellation. 

 

Neither your medical details nor your contact details will be shared with anybody else and will be treated as confidential. 

 

Balanced Massage would like to contact you occasionally via email or Facebook with details of new classes, extra holiday classes and a newsletter each term. We assume if you have supplied your email on the form that you are consenting to this.

If applicable you will be given the option to join a Facebook Group to which relevant posts regarding classes cancellations, timings, venue changes are added. You can remove yourself or asked to be removed by the Admin from any Facebook Group at any time.

A paper or digital health form is to be completed by all clients wishing to receive massage therapy. 

 

Your contact details are collected so that Balanced Massage can contact you to inform you of any changes, queries or re-scheduling to your appointment.

 

Your medical information is collected only so that Balanced Massage is aware of any medical conditions or contraindications that you have that may impact your treatment plan. We are required by law to keep these records for a minimum of seven years after your last treatment.
